I also noticed that imu was asking who the king of the country is, but shouldn't they know that there is no king of elbaph? I'm thinking this entity is something different from the imu we usually see. I think it's more of him demanding that the country needs a new king. Without any form of government the giants are just insects without unity and can't be put under the rule of world government.

The 5 elders were worried for him not because luffy was strong, but for something else, and he coughed blood when arrived on elbaph. He lived 800 years, but apparently he cannot stay at 100% all the time. He probably will run away from luffy and loki for this reason, because he would win both in normal 100%, but outside of castle he is dying and any time lost is painful for him. 

I am wondering if this is a heaven/hell thing where imu's forms change actual personas. Like in mg he is the ancient king and on the surface he is the devil, and the body just can't handle the power output for long like how luffy shrinks and grows old from g5. Maybe it is like an extreme version of blackbeard's fruit. Like he is completely immortal and regenerates but feels extreme pain or only takes "real" damage every time he uses his power. It's basically a deal with the devil type power where he is basically unstoppable but has to inflict grievous pain on himself as a price.
